New Reporting Form Instructions - September 2005Quarterly Progress Reporting CWMTF is implementing a new standard reporting form for its grants. The form should be utilized for the detailed narrative progress reports that grantees are to submit to our office every three months on each of their projects. The intention of the form is to help grantees facilitate their reporting to CWMTF. The top of the form should be completed with the reporting period (the three month period covered by the report), submittal date, grant number, grantee contact information, contract effective and end dates as well as the grantee's current anticipated project completion date. In the middle portion of the form, grantees should note the status of the project's schedule and cost. The bottom portion of the report form is for the narrative progress report. This is reserved for the standard detail that grantees provide on the progress they are making on each project. Grantees should attach additional pages as needed in order to provide as much detail as possible. The narrative report should also include information on any problems encountered and resolutions to those problems. Grantees should also attach any additional information helpful in demonstrating the status of their projects such as photographs, charts, etc. The report should be signed by the authorized signer for the grantee. Final Reporting The report form is also to be used as the cover sheet for final reports on projects. This report form does not replace any final reporting requirements outlined in the grant contracts. Those requirements should still be followed. This form will only serve as the cover sheet for those final reports. |
